We record the number with your account. If we need a copy of a certificate, we will ask for it by email — please don’t send documents through this form.

2. Order in your practice portal

Once your account is approved, we email your practice a sign-in link. Your portal has the order form, your prices and tiers, your orders and invoices — for your practice only.

Payment is by ACH. We email an invoice; the order is placed once payment clears. Ships direct to your practice.
